Midlakes Hold Harmless Agreement
Every
Midlakes Swim club is required to have this form signed by each participating
swimmer for the current season. The club is required to have this
information on file for the duration of the season.
Hold Harmless Agreement - WORD DOCUMENT
This
document can be used if you chose to modify it to include club information.
You must, however, forward the altered document to the League
for approval before distributing it to your club members.
